Delivery orders totaling less than $15.00 will have a $1.00 fuel surcharge added. 

Requests must be made by midnight prior to your scheduled pick-up day.

Chrome, Firefox, Opera, and Safari browsers will incorrectly show an error when this form is submitted

 and may also generate the request list.  We're sorry, but Microsoft, Google, Firefox, et. al. point fingers at each other,

claiming their product is not at fault.
